i suspect this could happen if we bootstrap pkg from an outdated /usr/ports (or
maybe a quarterly version), then later try to install packages from pkg.f.o
(wifi firmware, etc.) which requires upgrading pkg before installing the new
packages.

the fix is probably to either install from /usr/ports, or from pkg.f.o, but
never both, but this breaks release on platforms that don't have a binary pkg
package...
