Still, I would be inclined to take a closer look at hardware. Check
connections, et cetera. 

Nits: 

1) the OS is slightly outdated, should be at patch level 2, you might prefer to
defer the update until after this bug report is triaged

2) probably in your kld_list, there's fuse; should be fusefs.
