This appears to be caused by "xhci(4): Ensure the so-called data toggle gets
properly reset." (245d5a65f5805864881e2601190e7783057d2768).

Cherry picking 610528736f3f0bf51f990dd93c5061a7a437e519 and
6d8c6b24ee0a0416204356a98e4e7606489894c5 fix it for me. Hopefully they will be
included in -RC6.
