I'd guess either hardware trouble, or attempt to read directory with REALLY many files (maybe 10000). 

Rarely showing up hardware troubles happens SOMETIMES, and they are hard to track. Badly termitated SCSI bus can give you lots of strange errors, and I still wonder how could I trigger an error by upgrading OS (and downgrading did not help) once in 1996 ;-)
