>Description:
My card and several others that were supported by the 9631
driver are no longer supported by the 9746 driver. From Xorg.log:
(WW) NVIDIA(0): The NVIDIA Quadro NVS 280 SD GPU installed in this system is
(WW) NVIDIA(0): supported through the NVIDIA 1.0-96xx Legacy drivers.
(WW) NVIDIA(0): Please visit http://www.nvidia.com/object/unix.html for
(WW) NVIDIA(0): more information. The 1.0-9746 NVIDIA driver will ignore
(WW) NVIDIA(0): this GPU. Continuing probe...

(WW) NVIDIA(0): The NVIDIA GeForce4 Ti 4200 with AGP8X GPU installed ...
>How-To-Repeat:
Update to latest nvidia-driver port while using a no-longer-supported
card. Reboot. Fail to see X come up.
>Fix:
There should be an entry in ports/UPDATING alerting users that the
9746 driver does not support all the cards that the 9631 driver does.
A pointer to a list of dropped cards (if you can find one) would be
ideal. Instructions on using portdowngrade and HOLD_PKGS could also
