Portname should be qt-mysql-plugin. With Pkgnamesuffix it is 
qt-mysql-plugin-mysql-plugin which is somehow doubled.
>How-To-Repeat:
Install qt-mysql-plugin and take a look into pkg database.
>Fix:
Remove PKGNAMESUFFIX-line from Makefile.
