Actiontech MD560RD 3.1A1 modem - please help!

Thank you in advance to anyone replies!

I am new to FreeBSD. I installed
FreeBSD 5.1 RELEASE #3 on DELL
Inspiron 4100: P-III 1000 MHZ, 256 RAM,
ATI 16 Mb video, 3C905C NIC, and
Actiontec 56 K modem.

At the moment I am trying, unsuccessfully, to make my
modem work. I spent some time on the Internet trying
to find the info but, unfortunately, found very little.
I will appreciate any help.

Please find below kernel file and some output files.

Sincerely,
Tim
