call setup takes two tries?

I've seen calls rejected with this message in the past with some Telcos when the local phone number is set incorrectly in the SETUP packet.

I would try removing the 'Calling party number' from your SETUP request. (This used to require a patch to isdnd although I haven't checked the source recently)
