 please send us the following information:
 
 uname -a
 dmesg (with verbose boot)
 fsck -y
 atacontrol mode ad6
 atacontrol cap ad6
 
 and if you're not running a GENERIC kernel, please show us your kernel
 config.
 
 Also please tell us exactly the cvs tag you have used.
 
 And please send output of `smartctl -a /dev/ad6' (requires smartmontools
 from ports).
