uhub_reattach_port:413: could not allocate new device!

I've stumbled across another problem with usb2 recently that seems to  
be a regression.  In the past with usb2 I was able to plug in flash  
drives after boot and they would successfully attach, now that is no  
longer the case.  I boot from a usb flash drive that is mounted as / 
dev/da0s1a and I have no problems accessing it.

error message:
