These patches work ok on
FreeBSD 5.4-STABLE #0: Mon May 2 02:26:10 CEST 2005 root at 82-168-79-254-bbxl.xdsl.tiscali.nl:/usr/obj/usr/src/sys/RENE

As I have a CS4281 (I do have some docs), there is no midi yet,
but the device shows up:
% cat /dev/midistat
FreeBSD Midi Driver (midi2)
No devices installed.
However, these kernel messages (if WITNESS is turned on) appear, before
the status is shown:
malloc(M_WAITOK) of "4096", forcing M_NOWAIT with the following non-sleepable locks held:
exclusive sleep mutex midistat lock r = 0 (0xc2502a80) locked @ /usr/src/sys/modules/sound/midi/../../../dev/sound/midi/midi.c:928
KDB: enter: witness_warn
Calling uiomove() with the following non-sleepable locks held:
exclusive sleep mutex midistat lock r = 0 (0xc2502a80) locked @ /usr/src/sys/modules/sound/midi/../../../dev/sound/midi/midi.c:974
KDB: enter: witness_warn
These are repeatable.
