System crash during destroy or lost (in case hdd problem) mounted ciss volume. If volume is not mounted during destroy - problem is not exists.
>How-To-Repeat:
1. using 'hpacucli' create volume.
2. make file system and mount it.
3. destroy volume

Do not destroy mounted volume(s) and replace bad drive ASAP for avoid losting volume.
