 Please try the patch in kern/102211 PR.
 
 This is because ICH9R BIOS set 0x01000000 mask at intel_raid_mapping->disk_idx[]
 of broken(disconnected) disk. I think.
 
 I checked in the environment with kern/95459 patch too, but I think it is not
 necessary for this problem.
