A new installation of freebsd 5.4 or earlier works fine, no problem at all. But when we patch the kernel (p5 or p6, possibly also older patches), the problem occurs that the network interface card is getting slow.
Ping responses are very unstable:

64 bytes from ip.ad.dr.ess: icmp_seq=5 ttl=64 time=0.322 ms
64 bytes from ip.ad.dr.ess: icmp_seq=6 ttl=64 time=3.365 ms
64 bytes from ip.ad.dr.ess: icmp_seq=7 ttl=64 time=3.285 ms
64 bytes from ip.ad.dr.ess: icmp_seq=8 ttl=64 time=2.952 ms
This is in the local network.
Maximum download speed seen: 4519 kBps
Sometimes it's only 2000 kBps. Without the patches 11MBps is measured.

It are Dell 1850 dual XEON server. It's not happening on only one server, but on all of our Dell 1850's.

At last, we also have Dell 750's, same controller (pro/1000) and em driver without having the same problem.

I don't know. It occurs on a Dell 1850 with a Intel pro/1000 network interface card after patching the (default) kernel.
