This is a dual PIII system on an Intel L440GX+ motherboard. Unfortunately,
I could not keep the system running long enough to run send-pr, so I am
cutting and pasting the boot information from the log file instead of
generating it live from the failing configuration. Sorry. It's the best
I can do.
There are three ATA devices (2 UDM100 HDs on primary ata2 and a CD-ROM
on secondary at ata3). The L440GX+ has onboard SCSI, but there are not
SCSI peripherals attached.

I booted my system with its two disks attached to a Promise Ultra 100 TX2.
The system performed normally until I attempted I/O intensive operations
(streaming audio and, later, a fsck of the disks after the first crash),
whereupon I received this error message:
ad5: WRITE command timeout tag=0 serv=0 - resetting ata2: resetting devices
At this point, the enter system locked up and needed to be power-cycled.
Upon reboot, the fsck caused the same problem (this time on ad4).

Boot the system with a device connected to an Ultra 100 TX2 controller
and attempt intensive IO on the disk (a fsck is sufficient).
